Purpose
Record the five pieces of practice-related feedback collected during your revalidation period and note how each one influenced your practice.
Current NMC expectation
The NMC requires five pieces of practice-related feedback over three years. Its feedback log is a recommended way to organise the evidence, not a mandatory form.
Form structure
This blank preview uses the same field labels as the reviewed source definition used for the downloads on this page.
| Date | Source of feedback — Where did this feedback come from? | Type of feedback — How was the feedback received? | Content of feedback — What was the feedback about and how has it influenced your practice? |
|---|---|---|---|
| Record when you received or reviewed the feedback. | Describe the source without naming a patient, service user, colleague, student, or another person. | Record the format in which the feedback was received. | Summarise the feedback anonymously and explain its effect on your practice. |
Prompt-by-prompt walkthrough
- 1
Use one row for each piece of feedback
The requirement is five pieces of practice-related feedback gathered during the three-year period.
- 2
Describe the source, not the person
Use a broad source such as patient feedback, colleague feedback, appraisal, survey, or report and remove identifying detail.
- 3
Record how it arrived
The log separates the source of feedback from its format, such as verbal, card, survey, or report.
- 4
Capture influence as well as content
Summarise what the feedback concerned and what changed, improved, or needs further attention in your practice.
