Why anonymisation matters
The NMC, HCPC, and GMC all require reflective accounts to be fully anonymised. The NMC's revalidation guidance states that reflective accounts must not include any information that might identify an individual - whether a patient, service user, colleague, or family member. The HCPC and GMC set the same expectation for CPD and appraisal reflections.
Identification isn't just about names. A combination of details - a rare condition, a specific date, a named ward, an unusual age - can single someone out even when no name appears. This is called jigsaw identification, and it's why anonymisation checks look at context, not just obvious identifiers.
Including identifiable information in a reflective account can be a breach of confidentiality and of UK GDPR, and could put your registration at risk. A quick check before you save or submit a reflection is always worth it.
